Please note: If you are looking to purchase the same shirt in different colours, sizes, etc , you will need to send an additional order form by email to: [email protected]
Example : Two golf shirts complete two separate order forms; however, for credit card information you just need to fill the information out on one of the forms.

To make a purchase, please:
- Print and fill out the order form below
- Choose a shirt and/or a hat style
- Pick your colour from the available selections
- Pick a size
- Choose the style of logo you would like and if offered, the location of the logo on the shirt
- Add a name if desired (extra cost)
- Total up your order
- Fill out the payment information at the bottom of the form
- Send the completed order form to: [email protected]
